registrationHOW DO I REGISTER ?

To register as a patient ask at reception for details or fill in the online registration forms.  Health services reforms mean that patients will no longer be registered with an individual doctor but with the practice.

You may, however, nominate your preferred doctor and a note of this will be made on your records.  We cannot, however, always guarantee the availability of your first choice of doctor.  Newly registered patients will be invited for a new patient health check at this time.


 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

Refusal Of Application For Inclusion In The List Of Patients Or For Acceptance As A Temporary Resident

 

The practice will only refuse an application if it has reasonable grounds for doing so, for example if the applicant does not live in the practice area.  Applicants will be notified in writing of the refusal and the reason for it.


PRACTICE BOUNDARIES

The practice areas are defined on maps within the surgery. These have been agreed by the Hampshire Primary Care Trust.  Patient paper medical records are held by The Whiteley Surgery.
