Patient Survey Results
Please click here for our recent survey results (Aug 2016)
Due to the change in clinical systems all patients will be required to re-register for online services. Please pop into the surgery with two forms of ID (one photo/one written) and we can process your new registration token for you.
Specimens for laboratory testing can now be dropped at Fareham Community Hospital as well as the surgery if this is more helpful for you as our patients. Please make sure the samples are fully labelled and the request form is enclosed.
We are fully aware of the concerns about parking at Whiteley Surgery and we are sorry that the space available is so limited.
Unfortunately, when the surgery was built the parking was deemed adequate for the size of the surgery, the adjoining pharmacy and the clinic.
As the surgery is leased the landlords along with the local council have been contacted and made aware of our concerns and our patient’s feedback and we continue to work with our Patient Participation Group to try and find a solution, especially since the increase in the yellow line areas in roads near the surgery.
Please note: That with effect from 1 July 2016, our extended hours will include Thursday evening. The alternate Saturday mornings will remain unchanged. Please ask Reception for further details if required.
On-Line Appointment and Prescription Service
The surgery now has an online appointment and prescription service. This has replaced the old system and you will now be able to book and cancel appointments online. To register for this service, please pop into the surgery with photographic ID or fill out the online form on the Prescriptions Tab (top left), you will then get an email when ready for you to collect and bring your ID in (at the moment this is taking up to one week).
Automated self check-in
We have an automated self check-in service available at The Whiteley Surgery. To alleviate queues at the front desk please use this service if you are just checking in for an appointment. If you have any questions about using it then please ask the receptionists, they will be happy to help.
The Facts about General practice
Please find below a link for a leaflet produced by Wessex LMC for patients with information about General practice.
Facts about General practice
Patient Participation / Reference Group
The practice has an established Patient Participation Group who meet with representatives of the practice 4-6 times a year to discuss any patient concerns around the services the practice offers. We also have a 'virtual' group of patients that we contact by email. Both these groups of patients are involved in the annual survey, susequent results and an action plan to enhance service provision.
Since the PPG are there to represent your views we would politely encourage that any concerns you have are written and addressed to the Secretary of the Whiteley PPG as the correct channel.
If you wish to be part of either of these groups please speak to reception or fill out the electronic application from this website.
Please note that receptionists have the ability to record telephone conversations should they feel they are being spoken to in a rude or aggressive manner. Recorded calls will also be used for training purposes.
Text service to patients has started! Have we got your mobile number?
The practice operates a text service via mobile phones to remind patients of appointments, for chronic disease recalls and for other health campaigns. If you would like us to be able to contact you in this way please follow the link from the bottom of our home page 'Text Message Reminders', complete the form and send online.
